This form is to be used if you would like to permanently be removed from the NHS Cervical Screening Programme (NHSCSP) due to ineligibility.

Your responses will be checked to ensure you are correctly ceased. Once you have been removed from the screening list, you will no longer receive correspondence from the screening programme except a letter to confirm removal (excluding radiotherapy to the cervix).
